When the evidence changes, the report should change clearly. We do not quietly rewrite material facts or confidence labels.

Factual errors, misleading omissions, broken attribution, incorrect confidence labels and material changes that make the original presentation inaccurate.
How we correct
Material changes create a new version with a correction or update note. Minor spelling and formatting changes may be fixed without a separate notice when they do not alter meaning.
Challenges and removals
Source, privacy and rights concerns receive a documented review. Public material may be removed or redacted when justified, while the private audit history remains intact.
